Nicknames are a type of title given to your character when he/she fulfils certain criteria, or by random chance while meeting the requirements for a given nickname.

Nicknames in CK2, being actually descriptive epithets rather than true "nicknames," appear after the character's given name, such as in "Jarl Erik the Heathen" or "King Harald Hårdråde". To get a nickname the character must be a count or higher.

Nicknames are only localized when received[1], which allows to use localization commands such as "The Sacker of [Root.Location.GetName]" since patch 2.3.
